The first step towards an evaluation is to arrange an initial consultation. Initial consultation is quite simple. You can download all the forms
necessary from this page of our website. Before you fill out the forms, you need to gather any and all medical test results you have completed in the recent past.
Completely fill out all the information required, as much as possible. Then email or fax the documents to us.
